First Time Payment
Select from the three Care Plans programs below.
SILVER
$59 Care Plan + $35 Sign up fee
GOLD
$79 Care Plan + $35 Sign up fee
PLATINUM
$99 Care Plan + $35 Sign up fee
- x=service(s) included in the monthly membership
- Services not marked have an additional nominal fee due at the time of service.
- Labs for the GOLD PlAN are limited to basic laboratory studies
- Labs for the PLATINUM PLAN are selective for advanced laboratory studies.
- All plans include 12 visits per year.
Persons younger than 25 years of age (older than 12 years) may become a member for $49/month with limited labs included in the monthly fee.
Paying for Your Care
We accept payment by all major credit cards using Payeezy for our online payment services.
If you would like to join the Strawbridge Health family, please select your Care Plan and sign up, by clicking the pay now button. Your total cost will include a one-time $35 fee to your first month’s payment.
You will be scheduled for the recurring fee at your first office visit.
